As salamu alaykum,
This was the answer given by Wael in the following post:
http://www.zawaj.com/askbilqis/swearing-by-the-quran/
1. Lying is forbidden in Islam. Taking a false oath is considered to be a very serious sin. It's not something you can do and then just say a few "astaghfirullah" and forget about it.
2. It's forbidden in Islam to swear an oath by anything other than Allah. In other words, when we take an oath in Islam we say "Wallahi" or "Wallah hil-atheem." ("By Allah" or "By Allah the Most Glorious"). We do not swear on the Quran, or on the Prophet (pbuh), or on the Ka'bah, or anything besides Allah.
So my advice to you is to make a sincere tawbah for lying, and for swearing on something besides Allah. Ask Allah's forgiveness and resolve truly from your heart not to do these things again.
For more information about this, I am copying here an article from the As-Sunnah Newsletter (qsep.com):
Swearing in the name of anything besides Allah is prohibited
An oath is a solemn appeal to Allah to witness one's determination, or to emphasize a matter by mentioning a glorified one in particular. Glorification and exaltation are due to Allah Alone, therefore, it is not permissible to swear by other than Allah. Scholars are agreed that an oath must be sworn by Allah, His Names or Attributes only, and that swearing by other than Allah constitutes Shirk.
Ibn Umar (radhi allahu anhu) narrated, 'The Prophet (sallallahu aliahi wa-sallam) said, 'Allah has prohibited you from taking an oath by your fathers. He who must take an oath, may do so by swearing in the Name of Allah or he should remain silent.'' [Agreed upon] Ibn Umar (radhi allahu anhu) also reported that the Messenger of Allah (sallallahu aliahi wa-sallam) said, 'He who swears by other than Allah, commits Kufr and Shirk.' [Musnad Ahmad]
It is minor Shirk to swear by other than Allah, unless the one, who is sworn by, is aggrandized by the swearer, to the point of worship, then it constitutes major Shirk. Such is the case of the grave worshippers of today. They fear dead people (whom they aggrandize) more than they fear Allah, to the point that if one of them is asked to swear by a wali, he would not dare to do so, unless he swears on a truth, but if he is asked to swear by Allah, he would do so even if he is lying!
Swearing falsely is prohibited
Abu Umamah Iyas bin Tha`labah Al-Harithi (radhi allahu anhu) said, 'The Messenger of Allah (sallallahu aliahi wa-sallam) said, 'He, who misappropriates the right of a Muslim by taking a false oath, Allah will condemn him to the fire of Hell and will forbid Jannah for him.' A person asked, 'O Messenger of Allah, even if it is something insignificant?' He replied, 'Yes, even if it is the twig of an Arak tree.'' [Saheeh Muslim]
Abdullah Ibn Amr Ibn al-As (radhi allahu anhu) said, 'The Prophet (sallallahu aliahi wa-sallam) said, 'Of the major sins are; associating anything in worship with Allah, disobedience to the parents, killing without justification and taking a false oath (intentionally).'' [Saheeh al-Bukharee]
Another narration mentions, 'A bedouin came to the Prophet (sallallahu aliahi wa-sallam) and asked him, 'O Messenger of Allah, what are the major sins?' He (sallallahu aliahi wa-sallam) replied, 'Associating anything with Allah in worship.' The man asked, '(What is) next?' He (sallallahu aliahi wa-sallam) replied, 'Al-Yamin al-Ghamus.' He enquired, 'What do you mean by Al-Yamin Al-Ghamus?' He (sallallahu aliahi wa-sallam) replied, 'Swearing falsely to usurp the property of a Muslim.' [Saheeh al-Bukharee]
Ghamus' means to drown. A false oath drowns one in sin. For this reason, it is called Al-Yamin al-Ghamus, 'the oath that drowns one in sin.' Is it open to expiation? Some religious scholars believe that it is, while others think it is not, because in their opinion, it is a lie, which is employed to deprive somebody of his wealth and property. For this reason, they say that in such case, one should beg pardon from Allah and arrange to restore the wealth and property to the victim. This seems to be the more appropriate course because mere repentance cannot remit this sin. It is necessary to rectify the wrong and compensate the victim. [See, Riyadh as-Saliheen. Commentary by Hafiz Salahuddin Yusuf]
Swearing Frequently or Swearing in Transaction is prohibited, even if it be true.
Abu Hurayrah (radhi allahu anhu) said, 'I heard the Messenger of Allah (sallallahu aliahi wa-sallam) saying, 'Swearing produces a ready sale for a commodity, but he will be deprived of Allah's blessings.'' [Agreed upon]
The Prophet (sallallahu aliahi wa-sallam) informs us in this Hadeeth that when a seller swears falsely concerning the worth of his goods, he may attain some benefit by selling a large quantity of it, but this is not a fair means of business. Even though, it increases the sale, it robs the business of its blessings. Therefore, even if a person is perfectly right in his statement in respect to his merchandise, he should not take an oath to influence his customers.
Food for Thought: If one's perfectly true oath robs the deal of its blessings, then how great is the sin of those who take false oaths to sell their goods?!!
Salman (radhi allahu anhu) narrated that Allah's Messenger (sallallahu aliahi wa-sallam) said, 'There are three types of people whom Allah will not speak to, and neither will He bless them, and for them is the severe torment. They are, (i) the white-haired old adulterer, (ii) the arrogant beggar and (iii) the one who has made Allah as his merchandise, i.e., he does not purchase except by swearing (by Allah), nor does he sell except by swearing (by Allah).' [At-Tabaranee with a Saheeh isnad]
The Prophet (sallallahu aliahi wa-sallam) informs us in this Hadeeth that there are three categories of people to whom Allah will not speak on the Day of Resurrection nor will He cleanse them of their sins with forgiveness. This is because they committed sins without any excuse or justification and the first of these three is the fornicator and adulterer, who continues to sin in spite of his advanced years and consequent lessening of sexual urge, and his knowledge that the end of his life is near. The second is the man, who behaves arrogantly towards people, though, he possesses none of the wealth, rank or privilege, which often lead men to behave arrogantly. The third is the one, who treats Allah's Name with impropriety, frequently swearing by Him, Most High, without cause or valid excuse. In light of the above, those who frequently use the expression, 'Wallahi' (i.e. by Allah) should restrict themselves from such loose speech.

Below is all the proofs from the Shakyh, to sum it up though. Your oath is binding because you swore on the quran; the word of Allah swt ( ie taking an oath with Allah swt). If you took and oath with anything else it would not be binding but you'd have to repent for that. As for breaking an oath :
“Allaah will not punish you for what is unintentional in your oaths, but He will punish you for your deliberate oaths; for its expiation (a deliberate oath) feed ten Masaakeen (poor persons), on a scale of the average of that with which you feed your own families, or clothe them or manumit a slave. But whosoever cannot afford (that), then he should fast for three days. That is the expiation for the oaths when you have sworn. And protect your oaths (i.e. do not swear much). Thus Allaah makes clear to you His Ayaat (proofs, evidences, verses, lessons, signs, revelations, etc.) that you may be grateful”
[al-Maa'idah 5:89]

Smoking is clearly bad for you, and you need to make a sincere intention to stop. Read the ruling below and here's a exsert from it:
Considering all that has been presented, it can be clearly seen that smoking is an evil among many others. It is not permissible to indulge in it, or buy and sell it, or even to offer it to others. It is incumbent on a person who is addicted to it that he must make all efforts and get whatever necessary treatment to stop it Sheikh Muhammed Salih Al-Munajjid (see full ruling below)

He swore on the Qur’aan that he would not go to more than one doctor, and he wants to break that oath
I have been psychologically ill for 15 years. I feel that I cannot breath; I cannot sleep if I do not use a sleeping pill either. I sometimes have anxiety attacks, for which I visit many different doctors. My wife became upset because of this; so I swore by Quraan to her that I will not go to more than one doctor. Afterwards I needed to visit more than one. What is the ruling on this?.
Praise be to Allaah.
Firstly:
We ask Allaah to heal you and make you well, and we advise you to regularly recite dhikr and read Qur’aan, and ask Allaah and turn to Him, for all things are in His hand, may He be glorified.
For the remedy for anxiety and stress, please see question no. 21677.
Secondly:
If you swore that you would not go to more than one doctor, then you needed to go to more than one doctor, there is nothing wrong with you doing that, but you have to offer expiation for breaking an oath (kafaarat yameen), because the Prophet (peace and blessings of Allaah be upon him) said: “Whoever swears an oath then sees that something else is better than it, let him do that and offer expiation for his oath.” Narrated by Muslim (1650). But you should tell the doctor about your need for the medication, and you should ask him about increasing the dose for you, and you should not take such a decision by yourself, because of the harmful effects to which it may lead in the future.
Thirdly:
The one who swears by the Mus-haf intends either to swear by that which it contains of the words of Allaah, which is a binding oath for which expiation must be offered if it is broken, or he intends to swear by the paper, which is an oath sworn by something other than Allaah and it is shirk from which he must repent and resolve not to do it again, but he does not have to offer expiation if he breaks his oath, because it is a haraam oath that is not binding. Please see the answer to question no. 98194.
And Allaah knows best.

Swearing on the Quran weather valid or not is no small thing.
From what I understand its a grave sin Wa Allah aálam.
Instead of swearing oaths on the Holy book read it and ponder its miracles.
The Quran a mathematical miracle by the grace of Allah.
WORD REPETITIONS IN THE QUR'AN
Apart from the miraculous characteristics of the Qur'an which we have looked into so far, it also contains what we can term "mathematical miracles." There are many examples of this fascinating Qur'anic aspect. One example of this is the number of repetitions of certain words in the Qur'an. Some related words are surprisingly repeated the same number of times. Below is a list of such words and the number of repetitions in the Qur'an.
The statement of "seven heavens" is repeated seven times. "The creation of the heavens (khalq as-samawat)" is also repeated seven times.
"Day (yawm)" is repeated 365 times in singular form, while its plural and dual forms "days (ayyam and yawmayn)" together are repeated 30 times. The number of repetitions of the word "month" (shahar) is 12.
The number of repetitions of the words "plant" and "tree" is the same: 26
The word "payment or reward" is repeated 117 times, while the expression "forgiveness" (mughfirah), which is one of the basic morals of the Qur'an, is repeated exactly twice that amount, 234 times.
When we count the word "Say," we find it appears 332 times. We arrive at the same figure when we count the phrase "they said."
The number of times the words, "world" (dunya) and "hereafter" (akhira) are repeated is also the same: 115
The word "satan" (shaitan) is used in the Qur'an 88 times, as is the word "angels" (malaika).
The word faith (iman) (without genitive) is repeated 25 times throughout the Qur'an as is also the word infidelity (kufr).
The words "paradise" and "hell" are each repeated 77 times.
The word "zakah" is repeated in the Qur'an 32 times and the number of repetitions of the word "blessing" (barakah) is also 32.
The expression "the righteous" (al-abraar) is used 6 times but "the wicked" (al-fujjaar) is used half as much, i.e., 3 times.
The number of times the words "Summer-hot" and "winter-cold" are repeated is the same: 5.
The words "wine" (khamr) and "intoxication" (saqara) are repeated in the Qur'an the same number of times: 6
The number of appearances of the words "mind" and "light" is the same: 49.
The words "tongue" and "sermon" are both repeated 25 times.
The words "benefit" and "corrupt" both appear 50 times.
"Reward" (ajr) and "action" (fail) are both repeated 107 times.
"Love" (al-mahabbah) and "obedience" (al-ta'ah) also appear the same number of times: 83
The words "refuge" (maseer) and "for ever" (abadan) appear the same number of times in the Qur'an: 28.
The words "disaster" (al-musibah) and "thanks" (al-shukr) appear the same number of times in the Qur'an: 75.
"Sun" (shams) and "light" (nur) both appear 33 times in the Qur'an.
In counting the word "light" only the simple forms of the word were included.
The number of appearances of "right guidance" (al-huda) and "mercy" (al-rahma) is the same: 79
The words "trouble" and "peace" are both repeated 13 times in the Qur'an.
The words "man" and "woman" are also employed equally: 23 times.
Will they not ponder the Qur’an? If it had been from other than Allah, they would have found many inconsistencies in it.
(Qur’an, 4:82)
The number of times the words "man" and "woman" are repeated in the Qur'an, 23, is at the same time that of the chromosomes from the egg and sperm in the formation of the human embryo. The total number of human chromosomes is 46; 23 each from the mother and father.
"Treachery" (khiyanah) is repeated 16 times, while the number of repetitions of the word "foul" (khabith) is 16.
"Human being" is used 65 times: the sum of the number of references to the stages of man's creation is the same: i.e.
Human being 65
Soil (turab) 17
Drop of Sperm (nutfah) 12
Embryo ('alaq) 6
A half formed lump of flesh (mudghah) 3
Bone ('idham) 15
Flesh (lahm) 12
TOTAL 65
The word "salawat" appear five times in the Qur'an, and Allah has commanded man to perform the prayer (salat) five times a day.
The word "land" appears 13 times in the Qur'an and the word "sea" 32 times, giving a total of 45 references. If we divide that number by that of the number of references to the land we arrive at the figure 28.888888888889%. The number of total references to land and sea, 45, divided by the number of references to the sea in the Qur'an, 32, is 71.111111111111%. Extraordinarily, these figures represent the exact proportions of land and sea on the Earth today.238
